The following items are required to process a Medical Cannabis Employee Application:

• A "headshot" style photo for the license
• An unexpired government-issued photo identification

Email the completed package to [email protected] or hand-delivered to 2000 14th Street NW, Suite 400 South. Once processed, a payment link will be emailed to the applicant if not submitted with the application. The license will be issued via email.
